Compensation:

--- The pay for this position is $39.40 per hour

--- Benefits are available to full-time employees after 90 days of employment

--- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Highlights:

--- This position is a contract assignment for 13 weeks with potential to hire on permanently based upon attendance, performance, and business needs

--- The required availability for this position is 32 guaranteed hours per week

--- 35 units per week expected

--- Must be flexible to cover any territory as needed, but biggest needs are currently in the following areas: 40218, 40219, 40217, 40213, 40047, 40228

Responsibilities:

--- Administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care

--- Work under the supervision of a Registered Nurse (RN) and coordinate care with the interdisciplinary team, as well as the patient/family

--- Assist in providing nursing services and identifying the patient's physical, psycho-social, and environmental needs

--- Participate in planning, coordinating, and implementing care to provide quality, cost-effective patient care

--- Ensure effective communication to patient/family, co-workers, and other healthcare professionals

--- Assist the patient/family in learning appropriate self-care techniques and assess their response to care

--- Utilize infection control measures that protect both staff and patient

--- Assist the RN and/or physician in performing specialized duties related to end-of-life care for hospice patients

--- Assist with admitting, discharging, and transferring of patients

--- Possible on-call duties

Requirements:

--- Home health experience required

--- Homecare Homebase (HCHB) highly desired

--- Current Nursing License in the practicing state

--- Valid Driver's License --- Current CPR Certification

--- Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting

--- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing

--- Ability to use electronic medical charting and time-keeping software (Homecare Homebase)

--- Reliable transportation and current auto liability insurance coverage

--- Ability to recognize and maintain professional boundaries

--- Ability to work in a safe manner and report unsafe conditions

--- Participation in annual in-service and performance improvement programs

--- Must read, write, and speak fluent English

--- Good and regular attendance required
