    • Licensure : Must have or be willing to obtain a Kentucky state license and DEA certification.
    • Experience : Board certification or board eligible within two years of completing residency/fellowship required. Additional Information
      • Credentialing Timeframe : 90-120 days
      • Travel/Lodging : Provided
